Accumulation of moisture inside the 13.8 kV main bus and insulation tracking caused another arc-flash incident on the 13.8 kV Indoor Switchgear. An annual preventive maintenance was scheduled prior to the arc-flash incident but due to some issues with the power utility schedule, the schedule was postponed indefinitely. If the preventive maintenance was performed as scheduled, then the incident could have not happened.
